The following forms are required for all swimmers and clubs. The Hold Harmless Agreement and Concussion information forms below should be included in your registration documents. The Midlakes Club Compliance Form needs to be completed and sent to the League Vice President. The Insurance/Champ Entry fees check ($9 x number of swimmers per team) and the most up-to-date team rosters should be forwarded to the League Bookkeeper. For the 2011 season, please mail this information by June 1st to:
